What we do:
The service supports individuals experiencing mental health issues. We aim to reduce the impact of mental ill health and social isolation, while promoting recovery, independence, and community resilience.
Who we support:
The service offers support to individuals aged 18 and over living with mental health issues in Ceredigion.
How we do it:
The service provides up to four hours of support per individual each week. Where required, additional support is offered via telephone contact, enabling timely intervention to help minimise potential crises. Support also includes advice and guidance on developing independent living skills, reintegration into the community, emotional and therapeutic support, opportunities for peer support, and appropriate signposting to other services.
Referral:
Individuals can self-refer to access this service. We also accept referrals from Community Mental Health Teams (CMHTs), GP surgeries, third sector organisations, local authorities, as well as care and support services.
