What we do
Abergorki Outreach Floating Support Project provides community-based, person-centred supported living for individuals with mental health conditions. As a Registered Domiciliary Care Service, it delivers tailored support to help people remain independent in their own homes, achieve personal goals, and improve wellbeing.
Who we support
We support adults aged 18 and over in Rhondda Cynon Taf with mental health and/or substance use issues.
How we do it
The service works collaboratively with mental health and substance use services, families, carers, and partner organisations to deliver outcome-focused support that promotes recovery and prioritises each person’s needs, aspirations, and wellbeing. Support is shaped around individual needs, building skills, confidence, and independence.
Referral
All referrals to the Abergorki Outreach Floating Support Project are made through Rhondda Cynon Taf County Borough Council via the Integrated Assessment Process. Referrals are typically submitted by professionals within the health boards Community Mental Health Team and/or Substance Misuse Services, ensuring individuals are appropriately assessed and identified as requiring support.
