What we do

Abergorki is a CIW-registered service providing enhanced supported living for individuals with mental health or co-occurring needs, including substance use. The project supports individuals transitioning from residential or ward settings to develop the skills needed for future independent living. It offers opportunities to build confidence, independence, and daily living skills. 


Who we support

The service supports adults aged 18 and over living in Rhondda Cynon Taf with mental health and/or substance use issues.


How we do it

Working in partnership with mental health and substance use services, families, carers, and other organisations, the service delivers outcome-focused support that promotes recovery, wellbeing, and individual aspirations.


Referral

All referrals to the Abergorki Supported Accommodation Project are made via Rhondda Cynon Taf County Borough Council through the Integrated Assessment Process. Referrals are typically submitted by professionals within the health board's Community Mental Health Team or Substance Misuse Services, ensuring individuals are appropriately assessed and identified as requiring support.
