What we do:
This service aims to support people engaged with the Mental Health Crisis Services of Cardiff and Vale University Health Board, through developing a person's independence and empowerment to make informed choices and decisions regarding their social circumstances and needs in their community.
Who we support:
We provide support to individuals aged 18 and over living in the Cardiff and Vale area, referred to us from the Crisis Teams of Cardiff and Vale University Health Board.
How we do it:
We have meaningful conversations about what matters most to the person and offer the support and advice to help, connecting individuals with the appropriate support providers in their community, while developing a person-centred approach for each person's support needs.
Referral:
To access this service you need a referral from the Crisis Teams of the Cardiff and Vale University Health Board.
