Who we support
We support unpaid carers aged 18 and over whose cared-for person is receiving inpatient or community hospital support within Pembrokeshire.
How we do it
The service supports unpaid carers during hospital stays and discharge planning by working with hospital staff, helping carers understand care arrangements, and ensuring carers’ views and needs are considered. Following discharge, we provide community-based information, advice, signposting, wellbeing support, and access to Carer Needs Assessments to help carers maintain their health and prevent further hospital admissions.
Referral
We accept referrals from all sources, including self-referrals, professionals, and third-sector organisations.
