What we do
The service supports people with their mental health by working with them as individuals. We focus on what matters most to each person, taking the time to understand their needs, goals, and aspirations. We provide support to help improve the person's wellbeing, independence, resilience, and quality of life.
Who we support
We provide support to individuals aged 18 and over, living with mental health issues in the Llanelli, Gwendraeth Valley, and Amman areas.
How we do it
Our experienced staff start with a personalised wellbeing conversation, which helps us build a unique wellbeing plan for each individual. This plan looks at all areas of a person's life, allowing us to offer tailored, person-centred support that is flexible and meaningful. Everyone who accesses the service is offered between six and twelve support sessions according to need, giving us time to make sure the wellbeing plan works for them, regardless of their situation or experience.
Referral
Individuals can self-refer to access the service. We, also, accept referrals from GP surgeries, Community Mental Health Teams (CMHTs), and third sector organisations via the Preventative Services Hubs.
