What we do

Adferiad Ceredigion Supported Living Service (SLS) provides a community-based, person-centred supported living service for individuals with serious mental illness. As a Registered Domiciliary Care Service, it delivers specialist mental health support across Ceredigion to enable people to live independently in their own homes.


Who we support

The service supports people in Ceredigion aged 18 and over with serious mental illness.


How we do it

The service focuses on maximising independence by supporting individuals with daily living activities, practical tasks, medication compliance, and developing coping strategies. Support also encourages individuals to engage with their communities and personal interests, building confidence and skills to complete activities independently and enhance overall wellbeing.


Referral

Individuals must have an assessment of their needs completed by a Social Worker, Community Psychiatric Nurse, or Care Coordinator, which is then submitted to the Local Authority and Health Board Panel for approval. For support with making a referral, please contact us on the details provided.
