What we do:
The Community Connectors Service is provided by Diverse Cymru, part of Adferiad. We recognise that people have diverse cultural and personal needs, and the service exists to understand and address these requirements. We help individuals access community groups and services to improve their wellbeing.
Who we support:
We provide support to individuals aged 18 and over, who experience isolation, loneliness, housing issues, debt, mental health issues, and need signposting in the Cardiff City & South, Cardiff Southeast and East Cardiff GP Clusters.
How we do it:
We offer a confidential, non-judgemental space to listen and help our service users to access the support they need.
Referral:
To access this service you need a referral from a GP from the Cardiff City & South, or Cardiff Southeast, or East Cardiff GP clusters. We, also, accept healthcare referrals.
