Who we support
We support unpaid carers aged 18 and over who care for an adult experiencing mental health, substance misuse, or addiction challenges within Conwy.
How we do it
The service delivers free Carers Assessments on behalf of the Local Authority in line with the Social Services and Well-being (Wales) Act 2014. Assessments focus on the impact caring has on a person’s life and help identify support needs, wellbeing goals, and access to relevant services and support.
Referral
People can access the service through self-referral or referrals from professionals, services, or other organisations. No third-party referral route is required.
