What we do:

The Conwy Phase Two service supports individuals who are homeless or living in temporary accommodations to gain control of their lives, access appropriate support services and promote independence.


Who we support:

The service supports individuals in Conwy aged 18 and over with mental health issues, who are homeless or at risk of homelessness, or living in temporary housing such as hotels, B&B's or leased accommodation.
 

How we do it:

Support enables individuals to maximise their independence through support to gain life skills and access relevant services, support groups and organisations.
