What we do:
Lake Avenue is a supported housing and domiciliary care service based for adults experiencing mental ill-health and complex needs. The service supports individuals to maintain stable accommodation, reduce crisis situations, improve confidence, and work towards recovery and long-term stable independent living.
Who we support:
The service supports individuals aged 18 and over in Denbighshire who are experiencing mental ill-health may struggle with independent living, maintaining accommodation and other daily living skills. Some individuals may also have additional complex or co-occurring needs.
How we do it:
The service provides 24-hour support within a safe and homely environment, helping individuals develop independence, manage their mental and physical wellbeing, access education, or employment, and become involved in the community. Additionally, Lake Avenue has floating support flats where individuals can move on to for the next stage of their independence.
Referral:
Lake Avenue can be accessed through referrals from the Pathway Team at Denbighshire County Council Housing Team. Professionals such as mental health teams, housing services, social workers, or other support agencies may help individuals access the referral pathway.
