What we do:
Llys Glan yr Afon is an intergenerational housing scheme enabling individuals to live independent and fulfilled lives in their own homes. Support is provided in a flexible way which allows individuals to maintain their independence and to live a full and active life.
Who we support:
The service supports individuals in Powys aged 18 and older who require support with daily living. Some individuals may have a variety of health and social care needs.
How we do it:
Llys Glan yr Afon provides scheduled care and support as well as additional to support to help individuals to stay in their home for longer.
Referral:
Individuals can self-refer or be referred by a social worker or family members.
