What we do
Pembrokeshire Mental Health Community Support service provides floating outreach support to individuals experiencing serious mental illness alongside housing-related needs. The service offers person-centred, holistic support aiming to help individuals sustain their accommodation, build independence, and prevent eviction or unnecessary hospital admission, while reducing isolation through community engagement.
Who we support
The service supports people in Pembrokeshire aged 18 and over who are homeless, at risk of homelessness, or struggling to maintain their tenancy.
How we do it
Support is tailored to individual needs and may include help with daily living skills, budgeting, safety in the home, mental health, relationships, and accessing education, employment, and health services.
