Ty Hafal provides temporary supported accommodation to individuals who have experienced a serious mental illness and need support to live independently and integrate back into the community, enabling them to lead a full and active life.
Who we support
The service supports individuals in Pembrokeshire aged 18-65 living with mental health issues who require housing-related support.
How we do it
We encourage occupants to have ownership and pride in creating a comfortable environment where they can learn and develop social and daily living skills.
Referral
To be eligible to live in Ty Hafal it is necessary to be under the Community Mental Health Team due to serious mental illness and requiring housing-related support. Professionals can contact the team on behalf of individuals to receive a referral form.
