What we do:

Tŷ'n Rodyn is an 8-bed supported accommodation service in Bangor providing emergency housing for men at risk of homelessness following release from prison.


Who we support:

Tŷ'n Rodyn supports adult males aged 18 and over the Gwynedd County who are leaving prison and are at risk of homelessness or rough sleeping.
The service supports individuals with multiple and complex needs, which may include mental health conditions, substance use, and other support requirements linked to reintegration into the community.


How we do it:

Support is delivered in-house by Adferiad staff and focuses on helping individuals resettle into the community.

This includes support to:
Register with a GP and dentist
Access relevant Adferiad and partner services
Manage finances and develop independent living skills
Build stability following release from custody


Referral:

All referrals are managed by the Gwynedd County Council Single Point of Access (SPOA) Team and submitted to the service manager.
Multi-disciplinary meetings (MDTs) may take place where a service user presents a high level of risk to staff or others.
